Dear Recipient,

Since you are receiving an honorarium, we require the following information to issue a T4A tax form, if applicable:

This information is collected solely to meet Canada Revenue Agency (CRA) reporting requirements and will be handled securely and confidentially. If your honorarium exceeds $500 within the tax year, a T4A form will be issued for your records and tax purposes.
